VHS Working Papers - Option 1

OPTION 1 - Online Method Instructions

Please follow these steps in order to obtain student working papers. Any questions can be sent directly to the student’s counselor or to Miss Cass at [email protected]

Follow the instructions below

Step 1: AT-17 NY State Application for Employment

A) Click on the AT-17 application link below. You should DOWNLOAD this unfilled pdf form to your computer FIRST. Then open up the file you downloaded with either Google Doc or a PDF reader of your choice such as DocHub (free to download) or Adobe Acrobat, etc. so that you can begin filling out the form.

B) Fill out Part 1 ONLY of the form, remember to “sign” the parent signature via the signing option in the pdf or Google reader. You do not need to check off what type of certificate you want – we will know what to do based on age.

C) RE-SAVE the file you just filled out to your computer – you will be uploading it in step 2.

Step 2: The Google Form

After you complete Step 1, you can fill out the Google Form linked below. You will be required to upload the AT-17 Application that you just created in Step 1 to the Google form here:

As noted in the Google Form above, NY State requires proof of a physical within the last 12 months. If the school nurse or athletic department does not have a current physical on file, you must provide a statement from your physician with the date and signature of the doctor who conducted your physical.

You may take a picture of your physical and email it to [email protected] (you may have to pick it up from your doctor’s office – they will not email me directly)... Or, you can have your doctor fax proof to 757-2306, if the school is open.

Step 3: Receiving the working permit

Once the AT-17 application and the physical have been verified, Miss Cass will complete the working card. This may take a few days if we are waiting for a physical. It will be mailed by USPS mail to the family’s address as indicated in the Google Form.
