Working Papers Instructions
Please follow these steps in order to obtain student working papers during the COVID-19 shutdown of Vestal Schools. Any questions can be sent directly to the student’s counselor or to Mrs. Rupert at [email protected].
1) Students, when you access the AT-17 Application for Emploment Certificate below, you must
a) click on the icon at the top right that says “Open Original,” and then
b) DOWNLOAD the original form to your computer BEFORE you fill it out.
c) After you fill out the form, RE-SAVE it to your computer so that you can upload it in step 2.
If you do not do this, you will
send us a blank form.
2) Fill out Google Form listed below. You
will be required to upload the AT-17
Application for Employment Certificate to the Google Form.
3) As noted in the Google Form above, you
must also provide proof that the student has had a physical within the last 12
months.
- The school nurse will verify if the physical was performed within
the past 12 months at school for a sport, or if there is a physical
completed on file. The nurse will
contact the family via the preferred email address if there is no
physical on file.
- If the school does not have a physical on file, you must request
that your physician email you a statement verifying the date of the
physical within the past 12 months. You will be required to email this
statement to Mrs. Rupert at [email protected]
before final processing of the working card.
4) The Google Form should be submitted
electronically. Once the Google Form and the Physical have been verified, Mrs.
Rupert will complete the working card.
5) Mrs. Rupert will mail the card to the family's address as indicated in the Google Form.
6) The student must sign the card when
they receive it in the mail.
Please Note: This process will take a few days for you to receive the
working card in
the mail.
