New York State Education Law requires that schools provide the following health screenings:

• Vision Screening for all newly enrolled students, including color perception, distance acuity, near vision and hyperopia; also students in Kindergarten and Grades 1, 3, 5, 7 and 11 will be screened for near vision and distance acuity.

• Hearing Screenings for all newly enrolled students, as well as for students in Kindergarten and Grades 1, 3, 5, 7 and 11.

• Scoliosis Screening for all female students in Grades 5 and 7; and all male students in Grade 9.

New York State Education Department Guidelines require all medications, including over-the-counter medications that are to be given at school, to have the following:

•     A written order from a physician stating the medication, dosage, frequency and the time the medication is to be administered. 

•   Signed authorization from the parent or guardian to allow the administration of the medication in school

Medications must be delivered in their original containers to the school by the parent or guardian; students are not allowed to carry medications to and from school.

Students who have a written order from their physician stating that they are self-directed and instructed in the proper use of their medication may carry emergency medications such as an Epi-pen, inhaler or insulin.  

Students interested in playing a sport must sign up in the Health Office. Announcements will be made approximately one month before the sport season starts. 

The following are required to play a sport:

•  Medical Health History Form - this is required for each sport. Students must turn in a completed form when signing up for the sport.

•  Current physical exam on file in the Health Office. If a student does not have a current physical on file, the nurse will schedule a sports physical for that student with the school doctor.
